Agreement

I understand that I am ultimately responsible for my participation in the activities, services, or any products related to or provided by Thomas J McGuire.

I, at this moment, voluntarily accept and assume all risks, including emotional and physical injury, that would be related to my participation with Thomas J. McGuire and any company he represents.

I understand that it is my responsibility to withdraw from any activity or movement if I feel uncomfortable or if there may be any chance of injury.

In a physical or emotional injury, I waive my right to bring a lawsuit against Thomas J. McGuire, any member of his family, or any company or group he may represent. This holds for all jurisdictions in every state in the United States and any country.

In the event of an emergency, I, at this moment, grant permission to Thomas J. McGuire to call 911 or notify the necessary contact person to assist with whatever may be required for treatment or to solve any issue.
